I am currently using OC 1.5.5.1 but I don't have any regions in Northern Ireland such as Belfast. Can someone please advise on how to fix this, I have a customer who can't purchase and need to get this resolved. Thank you very much in advance for any help I receive.

Northern Ireland comes under United Kingdom. Select UK as country and you should see the NI regions - Londonderry, Down, Fermanagh etc.

That's strange. When I choose UK I can see County Antrim in the list. And all the other NI counties too.
I'm using the unaltered default installation.
If you check under system->localisation->zones, and go approx page 187 you should see NI counties at the end of the UK section (they are not all in alphabetical order).

You have stated you need Belfast as a region, but this is a city.
The address runs in the format road, town, city, county.
You could get around this by adding Belfast as a county in the database altho its a city.
TBH this is a bit messy because if you integrate with shipping api they will not take Belfast as a county and you'll end off reverting.
