Shipping How-To Tutorial?

Are there any tutorials around explaining how to put together a shipping module? I know PHP pretty well I just need a direction.

That is unless someone knows of a module that can handle these rules.

Best advice would be to pull apart an existing one like the weight based shipping. That one already supports multiple zones and ranges by weight. You can also read this to understand how zones and mapped to shipping rates
