Duplicating products in customers order

Hi - I have an intermittent issue with some customers orders. Every now and again an order will come in and each product that has been ordered has been duplicated on a separate line so the customer pays twice as much as they should. I have attached a screenshot of the latest order to do this. I am using 3.0.3.8.



Has anyone had a similar problem? Can someone suggest what may be causing this issue?

Unfortunately, this hasn't fixed the issue. We've just had a sale through that had 12 products which were correct and then two products that had been duplicated

What changes did you make? What theme and extensions are you using?

I'm using the default theme which has been edited using Live Theme Editor/ZenEditor for colour changing and I have the following extensions installed:

Complete SEO Package
Live Price Update
Mega Menu
Owl Slideshow
EU Cookie Law Notification Banner
Add to Cart Button Change and Pop-up Notification
Elfinder - Pro Image Manager 2.8.1
Fake Bestsellers etc
Product Quick View
CKEditor Full v.4.22.1

PayPal Checkout Integration
Stripe Payment Gateway

Did you make the changes exactly as the ones at the link below and then refreshed modifications?

Do any of those extensions modify the checkout page?

In you last test, when the products duplicate do you see 2 separate products in the cart like your screenshot or was the quantity just doubled?

Yes.

I don't think so, apart from Pay

Yes.

I don't think so, apart from PayPal and Stripe but I've used both of these on other sites with no issues.

It was two separate products just like the screenshot. The only difference was it only duplicated the last two products in the order, the first 12 products were fine just the last two were duplicated
