When refreshing the modifications cache, I'm getting an "HTTP Error 500"

We will be here for weeks trying to help you at this point and still be no closer - or maybe a wild guess. You make no mention if this is a new install, an upgrade, it has been running fine for x years.
Please explain your situation in full details and if you can not do that you will need to get a professional.
If you are feeling lucky -
You can disable every modification and maybe you will find one that causes the error.
You can return to default theme, maybe that.
You can check you have DOMDocument enabled on your server.
You can check your php version is compatible with your site.
You can check if you installed an official OC version (ie not use your servers "softilicious" or similar unofficial version of OC).
You can check your config files are pointing to the correct modification folder.

That turned out to be correct! I had the server guys to enable DOMDocument, and then everything worked! Question is why they disabled it.....
