When an affiliate registers an errors come up.

When an affiliate registers and these errors come up.
It was a fresh install last year so can only assume it has been doing it since but was only informed a few weeks ago. I have looked on git but couldn't find anything that worked. I am using 3.0.3.9

any help appreciated.

Works fine with OpenCart 3.0.4.0, successfully tested the registration of a new affiliate.

Could be a theme issue. Try with the default theme.
