I'm not often on the forum, however I'm trying to get hold of Tom and he's non-responsive via email.
I see he was last active 2 years ago on the forum.

any idea where he is?

We are having issues with his Moneris 3DS v2.0 opencart extension...

The module is putting orders through to "pending/payment received" when the card was declined, or if the customer made an error in the card and resubmits it's not even sending it to moneris with a "duplicate order number" error but it's still approving the order. Now we are having to manually contact customers to get payment information. We have reached out dozens of times and never get a reply, and this has been going on for 2 years now. We can't keep going on like this. He's forcing our hand off Opencart and to another platform.

Before we could revert to his older Moneris module to get by but Moneris killed the old API in July and forced us onto 3DS v2 and since then we've been dealing with this issue. For $140 the absolute least he could do is acknowledge customers.

Qphoria sometimes does reply to emails but doesn't ever check the forum. If he's not responding to his contact form though then I would guess he's had some personal issues, or moved on from OpenCart.

If that's the case, your best bet is to hire someone to look at and fix his extension. I understand it's a pain to pay someone to fix a commercial extension, but it's probably your only option at this point (unless some developer is interested in creating a new Moneris extension).

I'm not taking on projects myself but there are lots of developers that could probably help you. If you need to find a developer, you can post a request in the OpenCart "Commercial Support" forum, which is checked by a number of OpenCart developers. You can also try checking out the OpenCart "Partners" area.

Yeah we've tried that too. We've tried his form, his skype, through Opencart, and his email and he hasn't answered us in the two years we've been trying to get the extension working. We've also hired people to try to get it working properly with no luck. We're leaving for Shopify because at least Moneris supports it, they cut Opencart support for security reasons. Had an Opencart site since 2015, but this was the final nail.

The first one yes, the developer also didn't answer.

The second one doesn't apply because it has to be updated to the 3DS v2 security they implemented.
