I assume you mean 3.0.3.8. Are you using any extensions?

Have you checked the OpenCart error log, the PHP error log and your server error log for errors at the time it last happened?

Go to your MySQL database and select oc_user_group then add this to the permission: https://pastebin.com/9YmeqAwu

The most simple way possible solving this f.... issue is go into phpmyadmin, go to the opencart database, look for user_groups select it than export it, rename the sql file userfix.sql and every time u have the issue because no matter what u do it wont go away, there is no permanent fix to it so whenever u have the problem just replace the table, select the table than drop, than import and select the file u backed up and problem is solved, just do this every time u got the problem, 2 min fix....
