Hi, as the title suggests, we've updated from 2.3.0.2 to 3.0.3.8 which was successful we then updated to 4.0.2.3 to comply with PHP 8. Upon logging in to the dashboard all of the extensions are missing (see screenshot).
I've created a new user with a new user group but still no extensions (also no extensions in the "add user group" function either), I've cleared the Theme and SASS cache with no luck.
Any help is appreciated.

The upgrade function will almost certainly fail, especially since OC 3 extensions won't work in OC 4. OC 4 is still subject to development. If a stable release, use 3.0.3.9 or 3.0.x.x, using a fresh install. They support PHP 8, too. Use the Export/Import tool or PHPMyAdmin to copy your old data to the new OpenCart, and then re-install your wanted extensions. make sure the latter actually support OpenCart 3 and PHP 8.x versions.

Transitioning from the standard OC 2.3, it is uncertain whether those extensions will be compatible with 3.0.3.9, as 3.0.3.9 is designed to support PHP 8.2, barring any inclusion of backward compatibility.
