Languages
Journal offers full multi-language support for text and images, however languages are not part of the theme, they are part of the core Opencart system and installed separately.

Journal will detect any additional languages installed on your system and will provide extra input fields for translatable text.

Well, as regards languages, there are a number of issues with Journal3, in quite a few places it uses hard-coded English language text strings in the controller files, which is bad. And as I said, Journal3 uses its own single-page checkout. So for your extra checkboxes to work, you'd have to modify the Journal3 framework. Probably best to post on the commercial support section of the OpenCart forum.
