I posted it as a New issue on GitHub.
Let's see what happens.
It's a long standing issue, sadly. https://github.com/opencart/opencart/issues/1435

Mostly it won't cause issues as they are no links for search engines to index. The trouble can start when you start reorganising categories.

Closed in 2014 - what - the old politicians trick of if you ignore it it will go away :-)
The real trouble would start when someone decides to create a little website containing 300 links to your with nonsense urls
search engines will follow those urls and when a page is returned they ARE going to index that url. You can't do anything about that in the long run. Short term measures yep, but if someone was persistent enough then you will have a load of nonsense urls indexed. Search algos WILL detect something is up and it is not out of the question it could effect search rankings. In fact you could make sure that was the case with the right urls

Even without rearranging categories, it is pathetic that I sometimes see in Google search. My products are listed in the results list with irrelevant category names. It is sad that no one has cared so much about this problem for many years. An objection can always be found and it is easier than solving the problem. But what’s really disappointing is that there are solutions to it but you have to pay for them. In my opinion, this is a bug and not a missing feature of the program ...

Can anyone recommend a specific add-on that will definitely solve this problem?

Don't know of any extensions that fix this. The problem is made worse by the fact the sitemap contains URLs that don't match the canonical one on the page.

Check your pages canonical URLs are correct (should just the page with no parent categories). Fix your sitemap so it matches the canonical URLs on your pages. https://github.com/opencart/opencart/pull/9365

Add redirects to any URLs that are causing real problems.
