I have an OpenCart store, version 2.2.0.0. I recently noticed that my store doesn't add shipping costs in the final step in the checkout and therefore customers are always getting free shipping. In Step 3 of the checkout process the option for Flat rate shipping that I set up shows up and if I select it and go to final step, Step 5 it's not calculated in the total sum. Does anyone have an idea what might be happening?
Did you configured the geo zones from the admin as well as the shipping cost from the admin shipping extensions module page? In addition, to set option to Yes for each shippable products from the admin - > catalog - > products page?

2.2.0.0 had various issues so I suggest you upgrade to 2.3.0.2 or even 3.0.2.0.
