Journal Theme Custom Contact Form

Running latest Journal on 2.3.0.2.

For those familiar with the theme, I'd like to create a custom contact us form on the product page, ideally to be in product tabs/blocks, to request a quote .
The theme itself provides options for adding the store contact form, basically anywhere I want, or to create a popup module, but I can't seem to create something that does everything.
Any thoughts?
I have contacted theme developers and they offered zero assistance.

i would advise to search the marketplace (extensions) for 'custom form' modules or similar. and find one that is compatible with journal.There are a lot available and most should be or the developers will probably help.

Of course, they did, because you're asking for custom job. You need to find a developer, willing to do a custom job for you. And prepare to pay for it.
