Extra shipping options (inside delivery, liftgate, etc)?

I'm setting up an online store that will be selling big-ticket, heavy items. Let's say big-screen TV's.

Besides the standard shipping charge, I would like to add optional shipping services that the customer could choose. For example, inside delivery, liftgate service, haul-away of the old item, etc.

Is there a way to set this up within the standard OC system, without using extensions?
