I am looking for ready solutions
A basic and important question:
Does anybody know an extension / tool to create forms which customer fill out online and send back by click?
It is important to me to provide an own form sheet to customer ( specified dimensions and graphics)
I have created samples in MS-word and I want my customer to see them, I want mu customer to fill out my form online and I want to receive this sheet technically unchanged.
Any ideas, any experience?

you cannot just create a Form, and especially not by use of MS-Word, and implement it into OC, exept, if you are an experienced Coder, understanding the OC Framework Setup, and capable to add a lot of Declaration/Definition/Language-Stuff, to the existing Software, possibly some DB-Content as well, to make it work.
I would create a regular HTML/PHP Form Page, and handle Customer Contacts the 'manual' way, or then, get an OC-Pro, to get it made, because, as far as I know, nothing such exists, for free, for OC, to possibly be just modified a little, to make it work.

thanks a lot for your informative comment.
I use opencart furthermore as an advertisement page where advertisements for my customer are added as a product.
Therfore I wanted to use ready online forms in a category but you brought me to an other solution.
Solution:
1) All forms (4 maybe 5) can be provided on the category page and a customer can choose one and simply download it.
2) The cusotomer fills it out on his computer and uses a special "File upload gap" to send the form back to me
3) After uploading the form using this online uplaod gap it would be good to set an automatic e-mail for customer including payment details.
I think " Form builder Pro" should be able to handle this.
