I Need "Extra Fields module" for Register / Checkout

Hello,

I am in need of having extra fields and options added for registration/checkout of customer info,
like a radio button to select if you are individual or company, then show a text input for "Tax ID",
if individual selected, else show "VAT #" text field, a drop-down of "Tax Office Regions" and
"Company Name" text input, if company was selected...

The logic and field types shall be created through the module administration.

The input, should not be saved as comment on the order or all mixed in some existing DB column/field,
instead, I will need these fields, to be appended as new columns in the DB scheme of opencart or a new table,
because these fields, will need to be used in queries, as I intend to link my website with automated ERP/invoicing
system of our company.

Please, let me know if you know such module ready for download, free or for sale, both fine.

Else, please suggest me the best option for making this happen.
