I found this extension and its exactly what I need except does anyone know how to add a little thumb image next to the products?

Best thing to do is to contact the author of the mod and see if they can customize it for you.

Running Opencart v3.0.3.9 with multi-stores and the default template from https://www.labeshops.com which has links to all my stores.
