I am trying to set up the following shipping method, but can't figure out how to do it. Hopefully someone here can help.

Standard Shipping:
• Weight: 0-150lbs
• Per order: $4.75
• + Per item: $0.75
• + Per lbs: $0.45

Expedited Shipping:
• Weight: 0-150lbs
• Per Order: $9.00
• + Per item: $1.00
• + Per lbs: $0.75

It's pretty simple. I just don't know where to set this up.

You'll need to get a shipping extension, such as my Formula-Based Shipping one, to do a combo set-up like this. In Formula-Based Shipping, you would set up the rates like so:

RATE #1
Title: Standard Shipping
Sort Order: 1
Multi Rate Calculation: Sum
Weight Max: 150
Rate Type: Item-Based
Cost Brackets:
- From: 0, To: 99999, Charge: 0.75, Per (#): 1
Final Cost: Single
Final Cost Add: 4.75

RATE #2
Title: Standard Shipping
Sort Order: 1
Multi Rate Calculation: Sum
Weight Max: 150
Rate Type: Weight-Based
Cost Brackets:
- From: 0, To: 99999, Charge: 0.45, Per (lb): 1
Final Cost: Single

RATE #3
Title: Expedited Shipping
Sort Order: 2
Multi Rate Calculation: Sum
Weight Max: 150
Rate Type: Item-Based
Cost Brackets:
- From: 0, To: 99999, Charge: 1.00, Per (#): 1
Final Cost: Single
Final Cost Add: 9.00

RATE #4
Title: Expedited Shipping
Sort Order: 2
Multi Rate Calculation: Sum
Weight Max: 150
Rate Type: Weight-Based
Cost Brackets:
- From: 0, To: 99999, Charge: 0.75, Per (lb): 1
Final Cost: Single

Johnathan,
I would need Rate 1 & 2 combined as one shipping option. Same with 3 & 4. Here's a screenshot of how I have it set up on another cart that I'm moving from. Expedited shipping would be the same, just different values.

Looks like that's going to work for me. I entered everything in like you posted and the shipping came up correctly.

The reason it works is because rates with the same Sort Order are combined using the Multi Rate Calculation setting. (You can hover over the yellow ? bubbles in the admin panel for help with the settings.)
