Reviews and returns not working

I using template 46-3 with several Vqmods in place....everything was working fine,
Return are coming in but notifications are not emailed to admin, is there a way I can change that.

Important!!!!
Reviews cannot be submitted, the form gets displayed in a tab, then we fill it out, pop up a "please wait" box then does nothing.......Can anybody tell me what is going on....I am sure someone has had this problem before.

most likely a problem with your custom theme, try defaulting back to default theme and see if it works.
