Contact form not working

Hi there,
Ive just gone onto the contact page on my site to check everything was working ok.

at the bottom of the contact form just underneath the message box the following is displayed:

Notice: Undefined variable: error_verification in /websites/123reg/LinuxPackage21/cl/ub/be/clubberclobber.co.uk/public_html/catalog/view/theme/default_am-dark2/template/information/contact.tpl on line 45

ive tried to do a test email which produces the following in a white page:

the email actually works though, which is strange! it just looks odd to my customers

can anyone shed any light on how i fix this, i have a few other little errors on the site but they arent causing any problems just yet.

Open your information/contact.tpl (from your template folder) and make the following changes:

on line 42, change $entry_verification to $entry_captcha

on line 43, change $verification to $captcha

on line 45 & 46, change $error_verification to $error_captcha
