Options not passing to cart

Ok so let's say I'm selling a t-shirt. I set up the following 3 options

color
-red
-blue
-green

size
-small
-med (+2)
-large (+3)

design
-design a
-design b (+$5)

Now let's say CustomerB wants a red, large, design a shirt... he adds that to cart...

However the only thing passing through to the cart is a red shirt. I don't know what size he wants... or what design...

Another anomaly, he adds that red shirt to cart (remember, the other options didn't pass to the cart) and pokes around a bit more on the site. Decides to come back later.

When he comes back, ALL options have disappeared from the cart and only "tshirt" (with no color, size or design) remains in the cart.

What is going on here? Please help!

if a customer has a cart in his account and you resave the products whle that cart is in hois account the options get lost.

OK that makes sense.... but it's doing that without me editing the products...

Also it doesn't explain why only the first option passes through to the cart... and the other important options don't.
