Multi cost/shipping extension

The policy of the eshop i'm prepearing has the following rules

- Total of more 100€ and payment made by bank transfer, paypal, or visa has 0 (zero) shipping cost - any zone
- Total up to 100€ and payment made by bank transfer, paypal or visa, has 5€ shipping cost - any zone
- Any total, and payment with COD has extra 5€ cost (so, an order with 50€ total, will cost +5 for shipping, +5 for COD = 60€) - some zones
- Any total, and payment with COD (for the rest zones) - will be informed for the cost


So, the client has to choose first the payment method and then the shipping method. Then the order-total will calculate the extra cost.

Any idea if there is something ready, or someone who can create it ?
