Qphoria's "Coupon Select by" Mod issue

Hi,

I purchased Qphroia's "Coupon Select By Category & Manufacturer (OpenCart Addon)" and for some reason it stopped working. As you can see in the attached photo the manufacturers & categories no longer populate into their appropriate boxes.

Any help would be appreciated.
