Undefined Index issue with Carousel

Whenever I enable the stock carousel, I get an error code that reads as follows:

Thank you dan-arclight for the potential fix. However that method was not quite necessary and could possibly cause other issues. The issue is simply a bug in the tpl file for 1.5.1.1
Here is the patched file to fix the issue properly.
unzip and Just drop it into admin/view/template/module

Using the listed carousel.zip does not solve my problem. Look at :

I now have enabled the carousel module and added a banner.

Ok using the carousel.zip in fact helped.

I downloaded the new file and copied it to the //admin/view/template/module

Then again you have to uninstall the carousel module (if already installed)
And after this it does not make the errors with missing layout_id anymore.

But why is this new zip file not included in the 1.5.1.1. or maybe a 1.5.1.2 release
