Customiseable contact page form

The following features for the contact form would be great.

1. Predefine the subject line in the admin control panel so we can set outlook rules to sort incoming emails and know where the enquiry came from
2. Ability to see which store the contact form was filled out in, so we can respond appropriately and so we know which store and product they are talking about.
3. Ability to add or hide fields on the contact page for example, we may want the customer to enter the last name and address etc

someone recently release a custom form builder mod which you could use to generate any kind of contact for you want.

Although it doesn't do #1 or #2. I'll put those on my list to add in the future.
