URGENT - Login box in the header.

I need to have a login box like this one in the header. http://aext.net/example/twitterlogin/

I tried to incorporate Qforia's login box http://theqdomain.com/ocstore/login_sid ... in_sidebox in the header but no luck.

All I need is the email and password fields in the header somewhere. The site I'm using has no sidebars so the header would be the only place.
