Are you using a Linux or Windows server? If you send me a PM with your FTP and Opencart login details, I can take a look at it. First of all, it seems you are using an older version of the Export/Import tool.
Secondly, you have renamed your modified header.* files to header_b.* so it didn't pick it up.
Download and install the lasted version from http://www.mhccorp.com/opencart/export-import-tool

To get started on the file, I exported a spreadsheet to use as a template but am puzzled by the absence of a product_id field. Shouldn't each record have a unique ID? Let me know if there is any documentation available that would help answer such questions.

Better use OpenOffice then. I can't fix MS Excel 2007 bugs! And reporting bugs to Microsoft is a waste of time.In Excel 2007, when defining the 'categories' column under the 'Products' tab, I get an Excel alert that says "Number stored as text". When I import, I don't see any of those products. However, the first 10 rows, which I imported manually, appear on the site AND appear to be stored as text. Is there something I'm missing?
I just tested it with Excel2010, using numbers stored as text, even added a new product with a filled-in value for among others the categories, and then re-imported it. All products, including the new one, showed up just fine both on the backend and frontend of Opencart.
I don't have MS Excel 2007.

I've used Excel for these types of imports for years and I can't see Excel 2007 being the problem. I exported the latest file using the Export/Import Tool, opened in Excel, and there are products that have defined categories. For example, under the "Products" tab, lines 70 through 99 all show category 35. I just don't get how it can be this complicated. I'm even saving the file as 97-2003 XLS. I'll give your OpenOffice suggestion a try, but don't think it will resolve the problem.
On your own server, products with id 70 to 188 belong to category 35 according to your database. When I export that to a spreadsheet, this is exactly what I get, too. And your category 35 (Australian Gold) is showing up just fine both on the Opencart backend and frontend.
Also, as I said, if Excel2007 isn't able to store numbers as text, ditch it, and get another bugfree spreadsheet software instead!
