opencart_v1.4.9.1

I'm in need of a form that will display at the top of checkout 2 page just above coupon section.

The form will have first name / last name / email / phone number with validation.

We need to collect this info for every person who is attending a party so the number of forms displayed will be based on the quantity selected on product page.

On occasion 1 person will be paying for 15 people so when that person selects quantity 5 on product page I need 5 rows to display on checkout 2 page.

first name / last name / email / phone number
first name / last name / email / phone number
first name / last name / email / phone number
first name / last name / email / phone number
first name / last name / email / phone number

If paying for 8 then 8 displays etc etc. Max people will be 15 if that matters.

Does the extra info the customer inputs get saved to the database??
