Hosting Problem

Step 1 - Database

What i need to type at database host ?, I typed hosting address but where i need to type hosting username and password ?(normally which we connect to mysql database user/pass)
I used hosting address for example: 78.90.98.09:2340:username:passward
and also I tried with domain, where is username and password option for to connect mysql server. i created db and i have given user name password for db. but problem is not able to connect my db.

use the word "LOCALHOST"

If that doesn't work, try "localhost" small letters
