Dear all,

After sifting through the threads here in the forum..

I'm still lost about how to add a Dropdown box for Subject Line and have this linked so that when the email is sent, it would display as the subject line of the email.

Also, I require a How did you hear about us? With some Checkboxes for selection, and a Others checkbox with an empty field to input.

Kindly enlighten.

I would also be interested in this or a even better, a Custom Form Creator.

What I need is a new form for customers to fill with information about their cars, required for me to give them prices for auto spare parts.

Is there any Add-on that can provide a custom form creator?

I'm using Open Cart 1.5.4.1 on http://www.importpieseauto.ro

There is no front end so its not very easily customisable by the user, but I am happy to set it up and then give some advice on updating the values in the drop down boxes.

I was unable to find variable sources for $email, $name, etc. found in the form code.
