how to remove some fields from address page ?

Hi,
Want to remove some 'my account/edit address' stuff. I managed to hide some fields, but wand to completely remove the lines & leave default country as singapore without being in a select box.
I've tried some of the methods I've found in other posts, but mostly create problems.
It's shoppica2 themed for those who wish to boycott assistance
There's also a funny FB likebox error on this page that isn't in the other pages- the 'like' & Facebook are superimposed.
