Custom field (Input type = Text and Number)

Enquiry:

I wish to add a Custom Field for customer to fill in the Name and Number they wish to fill in.
At the same time, the price will be added according to their input text

For example :
$1 per Letter
$9 per number

Customer add their name and number DAVID #10 which will add the amount of money to the cart as well as the shirt they purchase

Means DAVID #10 is ($1 x 5) + ($9 x 2) = additional $23

Is there a way to add it to the cart attribute?
