RMA/Returns System ideas?

Hello,
I'm about to start writing the last thing I need for my sites (for now) - the RMA system.
I've been doing alot of thinking on how to do this - there is alot of ways to do it, but it has to be simple and easy for the customer. I decided to make a button under each invoice (order history), that links to a new form.
The new form displays a list of products with a checkbox - when checked it will display new input fields the customer will have to fill out.
But.. I would like some input from other people - do I need anything? Any better way to do this?

Sorry, not done with this yet. Been very busy the last few weeks. I'll see if I can have something ready for next week, but can't promise anything!

I think I can release it one of the following days.
I never really thought more about releasing it as a contribution, as I went from 1.3.2 to 1.3.4, and had alot of work to do. And hopefully 1.3.5 soon.
What version are you using?
