Product option with multiple adding price ?

Hi mate,
can you guys point me a direction how to make product option for :
sellilng T-shirt $ 10
Decorations :
embroidery $ 3.00
DTG $ 5
Vinyl $ 5

When client pick Embroidery there will be a set up fee of $ 40
so total will be $10+$3+40

set up fee can be add up on check-out doesn't matter or on the product.

May be you need an option level fee. I am not sure there is one ATM.
