It depends just how custom they are. Most themes that don't just copy the whole theme folder over, you should be pretty safe with, where as crappy overblown themes like shoppica will definitely screw up until they get updated
You can achieve a lot just by changing the stylesheets so when 'developers' just copy the whole default folder, It's actually quite baffling.
header.tpl is basically all you need and anything that needs tweaking you just copy the relevant file
