Custom orders

My client wants a custom order page that will allow buyers to decide on different options and then automatically add to cart. Is there an extension for that? Not sure where to look or what it might be called.

Theirs is a little more complex though. Pick the first sign in the list of products, and pull it up, like this one:

As you add letters, it adds the per-letter charge and updates the price, looks like jquery.
