If you have weight on a product, then using the included Weight-Based Shipping you can set a simple rate for each zone:
USA ZONE: 9999999:2.95
UK ZONE: 9999999:4.95
AU ZONE: 9999999:6.95
etc.
Or, if you'd rather price it by item count or a percentage of the subtotal, you can use the same Geo Zones with my Zone Plus Shipping module (at my site below)

2. If you have the UK VAT zones there, and you don't need them. Delete them
3. Click insert and make a new zone called "Local Rate Shipping"
4. Add the country & zones that you want this to be set for. (Your local area)
5. Goto Extension->Shipping. Edit the "Weight Based Shipping"
6. In the "Local Rate Shipping Rates" box, enter your values for local shipping.
7. Now any customers checking out from the country and zones that you set on the Geo Zone will see this shipping option.
Repeat steps 3-6 for each shipping zone

Then you disable flat rate and do it the way I said.
As far as doing it for every single country... This is an unfortunate plague of the current GeoZone system that has been there even in the old days.
I've thought about the best way of handling it but haven't come up with one.

Say for Germany I want to use table based shipping rates, the customers pay 19%VAT and they can use all payment options.
For french customers: I use UPS module and the customers pay 19% VAT and I dissallow payment with COD.
For customers from Switzerland: I use UPS module and the customers pay 0% VAT and I dissallow payment methods like Cheque or Paypal.
That would mean I have to create two Geozones for Shipping (UPS and Table Rate).
I have to create two Geozones for Tax (19% and 0%).
And I have to create 4 Geozones for the different payment options.
Does that work?
