Need Help With Shipping

Somewhere, I am messing this up..
I have shirts and hoods I sell, the hoods require shipping cost and the shirts are fixed rate.

When I put them in the cart this is what happens.

When, I put one item in the tshirt it works.

Yes, I am using the restrict shipping Methods by Product extension. I enabled it but, didn't mess to much with it. As I was trying to get the rest working first..

I followed the step to a T on GeoZones [for Dummies]

and now I can't get it to work...

I disabled your add on.. Until, I can resolve my issue... Maybe I need your Multi Flat Rate Shipping..

All, When I sell item individually it works fine.. The minute you put multiple items it does not add them correctly.
Example:
1. Hood 500.00 Shipping East or West Coast
2. Tshirt 20.00 Shipping Flat Rate

When I put Per Item it it works.. But if I put 1 hood and 1 Tshirt the shipping is way to cheap so I am removing that.
