[MODULE] complete detailed order lists

i'm willing to pay someone to develop a module where a printable page will display (in 2 columns)
Order ID
---------------------
Customer Name
Customer Address
Customer City
Customer Postcode
Customer Region/State

Customer Email
Customer Phone

Order Status
-------------
Products
-------------
Product Name:
Product Quantity:
Product Option:

this would be a simple module as all that is involved is retrieving the data from the database and displaying it.
Module MUST have a template which can be editable at a later date (so i can change the layout)

I want to be able to go to Reports -> Order List which will then display the page
and be able to filter the list either by Order Status or Date Added
this will then provide a list of all orders which can be printable to give to our warehouse.

module MUST work with version 1.5.0 with the ability to also work with 1.5.1.3
