How could I approach adding another checkbox item (just like the terms and conditions checkbox) during checkout which would require customers to agree to an additional disclaimer? I would like to place it right next to (above or below) the terms and conditions checkbox.
Ex: I would like to add a checkbox saying that the customer agrees that the they understand that shipping shown is only an estimate and may vary upon processing their order and they will be contacted for authorization, etc...

Adding something else for users to tick may confuse some. A better alternative would be to add it to the checkout T&C's
You can create a simplified version of your T&C's and assign it to Checkout in your settings.
Give the info page you create a sort order of -1 so it doesn't show in your information section as a link
