Creating another Contact Form

Is there a way to create another form (other than the contact form) and have it to where I can link to it in a page that I created inside "Information?" This form is going to have different areas than the contact form, and just the contact form will not work.

I am using the newest 1.5 v of Open Cart.

A simple way to do this would be to:

Create a new page, 'Edit' through Admin panel, click 'Source' on top of the the text box and paste your contact form code there then hit save.

Upload any additional files that are required to process the form and link them via editing 'header.tpl' in 'catalog->view->theme->yourtheme->template->common'
