Force Low order fee - second setting?

There should be TWO settings for this: one to set the fee itself (i.e. $5.00 penalty for orders that do not meet the minimum), and one to set the minimum threshhold (i.e., orders under $50.00 receive the $5.00 surcharge).

I've set my Order Totals low order fee to $50,000, set a Sort Order of 1 and set status to Enabled, but still cannot force it to apply to an order. I have searched forums and Admin. Note this is not Minimum Quantity, which is set on each product info page Data tab. This is a minimum dollar amount for an order.

Where is the mysterious second setting?
