On selecting insurance, pressing update gives blank screen

Hello all,
I need to trouble shoot an issue I am having with my opencart website. I am running 1.4.9 opencart. This issue just started about a week ago. When a customer is checking out and selects the insurance option and then click on the update button to the right. The screen goes blank. If the customer clicks on the back button the insurance is added to the shopping cart. Any ideas?
