Has anyone managed to do something similar to this using add-ons, modules or extensions in opencart?

I am using the latest version of opencart 1.4.9

I have a client who requires something similar. The main criteria being able to add multiple options per product. The addition of each option has cost implications, the price would either increase or decrease.

If this has not been done before I am happy to pay some one to build the mod.
