Flat Rate Shipping

I've mulled over some of the discussions on shipping but none told me quite what I need to know (unless I missed it). All I'm wanting to do is set up 3 shipping charges:
Free Shipping on orders of $100.00 or more
$5.00 Flat Shipping rate on orders $35.00 - $99.00
$5.50 Flat Shipping rate on orders up $34.00
Can someone please point me in that direction?

I don't think there is a price-based shipping module yet. I did wonder if the zone-shipping module could be modified to switch between total weight and total price
like
weight:cost
5:2.00, 10:5.00
or
price:cost
34.00:5.50, 99.00:5.00
but that still assumes that as the price goes us, so does the shipping cost. Would need additional handling for lowering the cost as the price goes up

OK, I suppose I have more mulling to do. Can I do just one flat shipping rate for all orders, regardless of weight or size?
Also, I haven't seen where I enable a shipping type to select when checking out. When doing a test order and clicking on continue after checking the address, I'm asked for a shipping type but one isn't there. What did I miss?

Yes Flat Rate will give you a flat rate for all orders no matter what. As long as the shipping geo zone is in the range of the customer geo zone. If you are using a fake address for your test account, it might not work. Also be sure it is enabled and your product "requires shipping" is set to YES

There is a subtotal based shipping extension for 0.7.7 in the contributions area.

I can be easily updated to work with 0.7.9, however, it too appears to suffer from the same incremental design that weight based shipping. Shipping Cost is expected to increase with increased total cost. Neither of them support decreased shipping cost as the quantity increases.
Probably not too hard to add. I'll take a look when I finish up my Authorize.net AIM module
