There was a similar error when I searched, and that was on line 6
I have the same issue. Ι Tried to update from 4.0.2.3 to 4.1. and stuck on "Applying patches..." for ever.

I use the stock theme, so it shouldn't be too hard. I have just changed some colors.
The buttons in 4.1 look wrong on the category pages, so that needs to be fixed first. It should look like 4.0.2.3
Probably a good idea as we are aware of some bugs.

Who knows why no one hasn't fixed the upgrade in any of the versions, or made a "how to" with a conversion chart that says use this tool for the products, categories, and customer accounts (ala export import tool), then for going from version X to Y you copy this table to this table with Z changes.

Whilst a fresh install may be best, it means that you have to go back & install all the extensions / language changes / etc - as well as re-keying any settings in extensions, copying customer / order / etc databases over - etc. etc ... The 4.0 upgrade tool worked perfectly until 4.1 came along - which implies that something was changed (directory??) in 4.1 - if such a change is needed, then is it unreasonable to suggest that a viable workaround is created at the same time to ensure that upgrades can continue to be made simply & effectively.

Is the "INSERT INTO" telling me what the issue is or how to fix it?? - if the latter, it just reproduced the error! - either way... how do I fix?
