My client's site experiences this issue 1-3 times a week, but understandably would like not have the problem as it takes time to reach the customer.
I noticed that JAY6390 has encountered this issue before, so I'm hoping he might have some insight into a possible solution.

Sounds like the sort of thing a developer would probably need to take a look at.

It's not even possible in OC 3.0.3.9 to add products to the cart when required options haven't been selected.
Hence, in order to help you here, you need to provide some more details, see forum rules.
To narrow it down, you have to examine the server's raw access log, to see how exactly the customer managed to to do a checkout for products without chosen required options. Perhaps there where spambots rather than genuine human users.

when I say "sometimes" I mean that a common thread that caused this behavior seem inconsistent. Most orders with options (there are many) go through, they print as they should, and all is well.
A few times a week, it soesn't work and we can't figure out why.
A couple of months ago, we hired Jay to settle some issues after the migration from OC 2.0.3.1 to 3.0.3.9 and thought that he might have some insight into the issue.

I wonder if this is at the root of the issue we are seeing, but I don't know enough about OC's internals to tell.
I have determined that this is a rare issue. It hints of a scenario where the customer adds a product to the cart, but does not complete the order and often goes away for a day or two and then comes back to complete the order.
I am aware that products with options cannot be added to the cart. It's the first thing I tested for. I'll see what can be gleaned from the raw access logs.

I have never experienced this and I work a lot on many different OC versions.
I did not experience it in OC1, specifically 1.5.5.1, my personal preferred OC1 version but if Jay says it exists, it existed.
That said this issue was fixed. I can not reproduce the error as is described in Jay's post on OC3.0.3.8 php8
What I can do is remove the option / edit the option and what was in the cart is a problem - but that would be correct.
If that is the problem then you will have to get a modification to prevent coming back and completing the order.
I am not sure why you need to edit an option, it will fail if you add a new option or remove the quantity of the old option.

There is a fix for 1.5.5.1 to remove products from the cart if an option no longer exists.
