This issue has been fixed already on the OC v3.2.0.0 release. This is caused by the promotional banner from OC that does not include a title.

To add one final note: this can also happen, if somebody has copied a controller file and renamed that (e.g. during a development process).
Then this "new" controller has - of course - no translation.
And as "placeholder" the "heading_title" is displayed.
To see which extension is cousing this, simply hover over the buttons and see which link is displayed.
