Product Duplicates Upon Save

Hi Team

When I edit a product and add options, and then save and go back to the product list, I discover the product has duplicated itself.
The more I edit and save, the more duplicates are created!!!
The edits appear in the duplicate product leaving the original as it was.

I am running OpenCart ver 4.0.2.2

im seeing something very similar on orders - anything that has options is randomly doubling the price on the orders placed. sometimes the order shows correct price but when you press refresh or confirm or even just f5 then products with options double or triple in price on orders.
