Product link to Categories with custom product title

Hello

we have products which we want to link to multiple categories. These categories are sub categories. At this point, its working OK but when displaying this product on front end, we want to show it with unique/custom title. For example

product name is ABC. Category names are X, Y, Z so output on the frontend should look like below

ABC is happy with X
ABC is happy with Y
ABC is happy with Z

any suggestion on extention or custom solution?
