Password Protect Your Front-End - Customizable

I purchased this extension and have not gotten a response for support after 2 attempts at submitting a ticket so I will check here.

The issue is that the entire password box is almost completely off the top of the screen. Any attempt to bring it down reverts back to the "50" "50" position.
It did work for a short time but now is useless.
Can anyone help? I had to disable the extension.
